Play Partners Clare Valley welcomes all referrals, and you can submit yours using the referral form below.

A referral from your GP is not required unless you are registered with the Chronic Disease Management Plan (formerly Enhanced Primary Care Plan) and wish to claim your session from Medicare.

NDIS clients can be referred directly, or via their Support Coordinator.
